Moelis: Request to Make a Change

Employees can add or remove insurance coverage during the Annual Enrolment Period in September, with changes effective 1st October each year. Outside of this period, changes are only allowed if you or your dependents experience a Lifestyle Change, such as: - Marriage, divorce, legal separation, or annulment - Starting or ending a domestic partnership - Birth, adoption, or placement of a child - Gaining or losing coverage under another employer-sponsored plan - Death of a spouse, domestic partner, or child
Making Changes If you experience a Lifestyle Change, submit your request and required documentation to the UK HCM team via this form within 30 days of the event. Effective Date & Processing Coverage changes take effect on the submission date, but processing may take 5-7 business days to reflect in all systems. Missed the 30-Day Window?
